As President Donald Trump’s administration continues to crack down on the widespread theft of taxpayer money, another massive fraud scheme has been exposed in California.
The Trump administration’s sweeping crackdown on government waste has uncovered yet another layer of financial abuse inside California’s benefit system.
New findings point to a troubling scam centered in Los Angeles County.
A fresh investigation has revealed that a single attending physician’s National Provider Identifier was linked to 17 separate hospice operations across the region.
Those entities collectively filed more than 3,000 Medicare claims for just 900 patients, billing taxpayers roughly $16 million.
Red Flags Ignored as Fraud Network Expands
Hospice care expert Ira Byock underscored how glaring the warning signs were.
“Anything over 100 patients at a given time that you have responsibility for as a hospice physician should start to raise red flags,” Byock said.
Despite those clear indicators, the physician and associated operations remain active in the Medicare system, with no reported disciplinary action or charges, highlighting a longstanding failure of oversight.
Federal authorities under President Trump have moved aggressively to dismantle such schemes.
The Centers for Medicare and Medicaid Services, led by Mehmet Oz, recently removed roughly 450 suspected fraudulent hospice providers in Los Angeles County alone, freezing more than $600 million in questionable claims, many of which were never appealed.

Billions in Fraud Tied to Organized Networks
Officials say the hospice fraud uncovered is part of a much broader pattern, with billions of dollars allegedly tied to organized criminal networks and systemic abuse of taxpayer-funded programs.
The latest revelations build on earlier federal actions targeting what has been described as a $146 billion Medi-Cal fraud network operating within California.
Meanwhile, critics point to resistance from state leadership under Gavin Newsom, arguing that efforts to expose fraud have faced political pushback.
Independent reporting has captured tense on-the-ground encounters, with providers shutting doors and avoiding scrutiny when questioned.
Patients and families have also spoken out.
One couple in Visalia summed up their experience bluntly:
“The way I see it, we were just taken in.”
Oversight Failures Leave Vulnerable at Risk
While California officials have emphasized that Medicare is a federal program and pointed to their own enforcement efforts, including a $267 million fraud takedown earlier this year, the scale and persistence of abuse in Los Angeles County continue to raise serious concerns.
The region has an unusually high concentration of hospice providers, far exceeding many entire states, creating what investigators describe as a fertile environment for exploitation.
The Trump administration’s Task Force to Eliminate Fraud is now working to shut down these operations, aiming to protect both taxpayers and vulnerable patients from systems that critics say have been treated as cash pipelines.
With billions allegedly siphoned off while seniors were enrolled in services they did not fully understand, federal officials argue the current crackdown marks a turning point, bringing long-overdue accountability to a system that had operated with minimal scrutiny for years.
