One of the world’s most prominent and influential euthanasia advocates is now urging the Canadian government to begin killing mentally ill patients, marking a chilling escalation in the country’s already controversial “assisted suicide” regime.
Eugenics advocate Jocelyn Downie, a longtime lobbyist for the euthanasia industry and professor emerita at Dalhousie University, made the case during testimony before Canada’s Special Joint Committee on Medical Assistance in Dying (MAiD).
Downie is demanding that the government approve euthanasia for mentally ill individuals in order to supposedly “help” them.
She argued that people suffering from mental illness will probably kill themselves anyway if the government doesn’t euthanize them first.
Push to Expand Euthanasia to Mental Illness
Canada is already preparing to expand eligibility for its euthanasia program, known as Medical Assistance in Dying (MAiD), to include people suffering solely from mental illness by March 2027.
Downie strongly supports the move.
“What will happen, if there is an extension or an exclusion, is that people will die by suicide,” she told lawmakers.
The argument has stunned critics, who say it effectively suggests the government should facilitate suicide rather than prevent it.
From Euthanasia to “Assisted Suicide” to “MAiD”
Downie has spent decades pushing for legalized euthanasia in Canada.
Her efforts have included involvement in the landmark Carter v. Canada ruling that struck down the country’s ban on “assisted suicide.”
Over time, the language surrounding the issue has shifted dramatically.
Euthanasia was softened to “assisted suicide,” then further to “physician-assisted dying,” and now simply “MAiD.”
Yet, critics warn that the shift in language softens the reality that the government is killing people with lethal injections.
Downie herself authored “Dying Justice: A Case for Decriminalizing Euthanasia and Assisted Suicide” and was later awarded the Order of Canada in 2017 for her work in the field.
Critics Warn of a Dangerous Shift
Opponents argue that expanding euthanasia to those with mental illness crosses a major ethical line.
They point to reasoning used in Carter v. Canada, where the court argued that banning assisted suicide could “deprive individuals of life,” a conclusion critics say turns basic logic on its head.
Since legalization, Canada’s euthanasia system has steadily expanded beyond initial assurances that it would remain tightly controlled.
Critics say what was presented as a limited, last-resort option has evolved into a system with increasingly broad eligibility and minimal enforcement.
Growing Alarm Over Future Expansion
Downie has previously co-authored research suggesting that suicidal thoughts should not automatically disqualify someone from seeking euthanasia.
That position has fueled concerns that the system could begin treating suicidal ideation as a valid justification for state-assisted death, rather than a crisis requiring intervention.
For families, the implications are deeply unsettling.
If the policy expands as planned, critics warn that individuals struggling with mental health issues could be approved for euthanasia, even in cases where recovery might have been possible.

Canada’s euthanasia program has already drawn international attention, with some countries slowing or reconsidering their own legalization efforts after observing its rapid expansion.
Now, with proposals to include mental illness, the debate is entering even more contentious territory.
